Güzellik Rehberi
Leke ve Melasma Rehberi: Tranexamic Acid, Azelaic Acid, Vitamin C, Niacinamide ve Retinoidler nasıl farklı?
Her kahverengi leke melasma değildir. PIH, solar lentigo ve melasma farkını; tranexamic acid, azelaic acid, Vitamin C, niacinamide, retinoidler ve sunscreen üzerinden mekanizma bazlı karşılaştırıyoruz.

“Leke serumu istiyorum.”
İlk sorum:
Hangi leke?
Çünkü consumer dilinde “leke” dediğimiz şey:
- melasma,
- post-inflammatory hyperpigmentation — PIH,
- solar lentigo,
- freckles,
- acne sonrası kahverengi iz,
- post-procedure pigment
olabilir.
Ve bunlar aynı biyoloji değil.
Bu yüzden bütün pigment serumlarını “brightening” başlığı altında toplamak kafa karıştırıyor.
2025 expert consensus melasma management’da broad-spectrum photoprotection’ın essential olduğunu, supervised hydroquinone-based triple combination’ın reference/gold-standard role taşıdığını ve azelaic acid, kojic acid, oral TXA gibi alternatives bulunduğunu vurguluyor. Procedures ise adjunct/refractory cases için ayrılıyor.
Kaynak: https://pubmed.ncbi.nlm.nih.gov/40996222/
Yani modern pigmentation strategy:
tek serum değil.
Diagnosis + photoprotection + doğru pathway.
Önce leke türünü ayıralım
Melasma
Symmetric facial hyperpigmentation.
Hormonal + UV + visible light + vascular/inflammatory/dermal factors.
Chronic.
Relapse-prone.
PIH
Inflammation sonrası pigment artışı.
Acne.
Eczema.
Procedure.
Trauma.
Özellikle darker skin tones’da prominent olabilir.
Solar lentigo
Cumulative UV-related discrete brown spots.
Freckles
Genetic/UV-responsive pigmentation.
PIE
Post-inflammatory erythema.
Kırmızı/pembe.
Pigment değildir.
Ve işte önemli nokta:
TXA, Vitamin C veya niacinamide “kırmızı acne izi” için aynı logic’te çalışmaz.
Pigmentation ingredient map
| Ingredient | Ana mekanizma | En mantıklı kullanım alanı |
|---|---|---|
| Sunscreen | yeni UV stimulus’u azaltır | bütün pigmentation |
| Visible-light protection | VL-triggered melanogenesis’i azaltmaya yardım | melasma/PIH |
| Tranexamic Acid | plasmin-related signaling | melasma/PIH |
| Azelaic Acid | tyrosinase + inflammation + keratinization | acne + PIH/melasma |
| Vitamin C | antioxidant + melanogenesis support | general uneven tone/photoaging |
| Niacinamide | melanosome transfer + barrier | mild pigmentation + support |
| Retinoids | turnover + signaling/remodeling | photoaging + acne + pigmentation |
| Hydroquinone | melanin synthesis inhibition | melasma/hyperpigmentation medical context |
Bu tablo “hangisi winner?” sorusunu daha doğru hale getiriyor.
Çünkü farklı pathways.
1. Sunscreen neden birinci sırada?
Çünkü pigmenti azaltmaya çalışırken:
UV stimulus devam ederse,
yeni pigment üretimi de devam eder.
Melasma/PIH literature broad-spectrum protection’ın central role’ünü destekliyor.
Kaynak: https://pubmed.ncbi.nlm.nih.gov/32029932/
Bu yüzden:
leke serumu + güneş kremi yok
çok zayıf strategy.
2. Visible light neden önemli?
2026 international Delphi consensus visible light’ın:
- PIH,
- melasma,
- bazı photodermatoses
üzerinde role sahip olduğunu ve riskli populations’da visible-light photoprotection consideration gerektiğini belirtiyor.
Kaynak: https://pubmed.ncbi.nlm.nih.gov/42101389/
Bu yüzden melasma için:
iron-oxide-containing tinted sunscreens
özellikle ilgi çekici.
3. Tranexamic Acid
TXA:
plasminogen/plasmin system üzerinden pigment signaling’i modüle eder.
2024 meta-analysis:
22 RCT,
1280 patient.
Oral, topical ve injectable TXA melasma severity’yi azalttı; oral route en büyük MASI reduction’ı gösterdi fakat heterogeneity yüksekti.
Kaynak: https://pubmed.ncbi.nlm.nih.gov/38843906/
Topical TXA:
consumer skincare için daha accessible.
Ama oral evidence’ı direct serum claim’e çevirmemek gerekiyor.
4. Azelaic Acid
Azelaic acid çok ilginç çünkü iki dünyayı birleştiriyor:
acne + pigmentation.
Mechanisms:
- tyrosinase inhibition,
- anti-inflammatory effects,
- keratinization modulation.
Bu yüzden:
acne + PIH
olan biri için çok logical ingredient olabilir.
Melasma systematic reviews azelaic acid’i evidence-supported topical options arasında gösteriyor.
Kaynak: https://pubmed.ncbi.nlm.nih.gov/38099013/
5. Vitamin C
Vitamin C:
antioxidant.
Melanogenesis-related effects.
Collagen-support.
Photoaging.
Ama topical Vitamin C evidence:
formulation quality’ye çok bağlı.
L-ascorbic acid:
stability + pH challenge.
“Vitamin C var” tek başına product potency söylemez.
Melasma systematic review’lar Vitamin C için promising ama daha variable evidence olduğunu belirtir.
Kaynak: https://pubmed.ncbi.nlm.nih.gov/38099013/
6. Niacinamide
Niacinamide:
melanosome transfer pathways
ve barrier biology ile ilişkili.
Çok iyi tolerated broad-support ingredient.
Ama severe melasma için:
tek başına heroic treatment değildir.
Mild uneven tone + sensitive routine’de excellent support.
7. Retinoids
Retinoids:
cell turnover,
epidermal remodeling,
pigment distribution,
photoaging
üzerinde etkili.
Melasma management’da retinoid-containing combination therapies çok önemli.
Ama irritasyon:
PIH-prone skin’de geri tepebilir.
Bu yüzden:
daha hızlı = daha çok irritation
iyi strategy değildir.
Hydroquinone nereye oturuyor?
Medical depigmenting agent.
Melasma için evidence çok güçlü.
2025 consensus supervised hydroquinone-based triple combination cream’i gold-standard reference olarak vurguluyor.
Kaynak: https://pubmed.ncbi.nlm.nih.gov/40996222/
Ama:
- regulatory availability değişir,
- medical supervision gerekir,
- long-term indiscriminate use uygun değildir.
Bu consumer serum rehberi değil.
Topical treatments için 2023 meta-analysis ne buldu?
45 efficacy studies / 2359 patients.
Hydroquinone, cysteamine, TXA, azelaic acid, kojic acid
meaningful efficacy gösterdi.
Irritation profile farklıydı; azelaic acid ve topical TXA daha düşük irritation rates ile dikkat çekti.
Kaynak: https://pubmed.ncbi.nlm.nih.gov/36566490/
Bu bize çok güzel bir şey gösteriyor:
efficacy tek karar kriteri değil. Tolerability de tedavinin parçası.
Melasma vs PIH: hangi ingredient daha mantıklı?
Melasma
Photoprotection + targeted depigmenting strategy + recurrence management.
TXA, azelaic acid, hydroquinone systems, retinoids, kojic/cysteamine gibi options context’e göre.
PIH
İlk iş:
inflammation source’u durdurmak.
Acne varsa acne treatment.
Eczema varsa eczema management.
Sonra pigment support.
Acne + leke varsa?
Azelaic acid çok elegant.
Retinoid de dual role sağlayabilir.
Niacinamide support olabilir.
TXA additional pigment pathway olabilir.
Ama 5 active birden:
irritation.
Red acne mark — PIE
Kırmızı iz pigment değildir.
Vascular/inflammatory.
Bu nedenle pigment serumlarını endlessly layer etmek başarısız olabilir.
Time + inflammation control + appropriate vascular options context’e göre.
Brown acne mark — PIH
Melanin.
Burada pigment-targeting actives daha mantıklı.
Solar lentigo
Discrete sun spots.
Topical actives yardım edebilir.
Ama certain lesions procedures (IPL/laser/cryotherapy etc.) ile daha direct respond edebilir.
Diagnosis important because all brown lesions cosmetic “sun spot” olmayabilir.
Melasma için laser neden riskli olabilir?
Melasma recurrence-prone.
Heat/inflammation bazı patients’da worsen/PIH oluşturabilir.
2025 consensus lasers’ı generally refractory cases için reserve ediyor.
Kaynak: https://pubmed.ncbi.nlm.nih.gov/40996222/
Bu “laser hiç kullanılmaz” demek değil.
Ama first reflex olmamalı.
IPL?
Superficial photoaging pigment için useful.
Melasma’da more cautious.
#44’te detaylı.
Chemical peel?
Adjunct olabilir.
Ama aggressive peel:
PIH risk.
Skin tone + depth + agent selection önemli.
Microneedling?
Drug delivery/adjunctive melasma studies var.
Ama procedure + topical combination outcomes’ı yalnız active’e bağlamak zor.
Leke rutini nasıl sade olur?
Sabah
- Gentle cleanse
- Antioxidant/brightening serum — optional
- Moisturizer — if needed
- Broad-spectrum sunscreen
- Melasma/PIH context’inde tinted visible-light protection düşünülür
Akşam
- Cleanse
- Tek bir main pigment active
- Moisturizer
Multiple actives:
slow introduction.
TXA + Niacinamide
Different pathways.
Good combo.
TXA + Azelaic Acid
Possible.
Ama irritant load monitor.
Vitamin C + Niacinamide
Modern formulations’da universal incompatibility yok.
Retinoid + Azelaic Acid
Possible.
Dual acne/pigment benefits.
Sensitive skin için alternating schedule practical.
“Dark spot corrector” alırken ne sorarım?
- Leke türüm ne?
- Active ne?
- Clinical concentration var mı?
- Sunscreen strategy var mı?
- Visible-light protection gerekiyor mu?
- Routine irritation yaratıyor mu?
- Before/after ışığı aynı mı?
- Melasma ise relapse planı var mı?
Hangi ingredient daha hızlı?
Bu soru consumer için doğal.
Ama pigmentation:
weeks-months.
Melasma:
chronic.
7-day brightening:
çoğu zaman hydration/optical effect.
Gerçek pigment changes daha yavaş.
Melasma permanent cure var mı?
Bugün için “kalıcı cure” dili riskli.
Control + maintenance + triggers.
Relapse common.
Hormonal melasma
Hormonal driver tamamen serumla ortadan kalkmaz.
Pregnancy/postpartum/hormonal medication context:
individualized.
Kısaca
Melasma için en önemli temel?
Photoprotection.
Kaynak: https://pubmed.ncbi.nlm.nih.gov/40996222/
TXA ne yapar?
Plasmin-related pigment signaling; oral/topical/intradermal evidence var.
Kaynak: https://pubmed.ncbi.nlm.nih.gov/38843906/
Azelaic acid?
Pigment + inflammation + acne overlap.
Vitamin C?
Antioxidant/brightening support; formulation quality çok önemli.
Niacinamide?
Melanosome transfer + barrier support.
Retinoids?
Turnover/remodeling/pigmentation; irritation management şart.
Visible light önemli mi?
Evet, özellikle melasma/PIH context’inde.
Kaynak: https://pubmed.ncbi.nlm.nih.gov/42101389/
En doğru tanım?
Leke tedavisi tek hero ingredient seçmek değil; leke türünü doğru ayırıp photoprotection ile doğru pigment pathway’ini eşleştirmektir.
